Billing Terms

As an SCBN customer, you are responsible for monthly dues based on the number of medications for which you receive assistance. A one-time application fee is added to each SCBN enrollment. This administrative fee is non-refundable. You may access your billing history and current payment terms at any time on your "My Account" page in your patient console. All charges appear on your statement monthly as SCBN. Access to SCBN may be cancelled at any time by the customer or the company. Your username and password may not be assigned of transferred to someone else. All materials and tools are provided "as is." There are no warranties or guarantees provided. Materials on this website are not to be copied or distributed without prior approval from the company. All notices to users will be sent electronically.

SCBN.org reserves the right to change the terms of this agreement at any time, without advance notice.
